JOB TITLE: Infectious Disease Nurse Coordinator

FLSA: Exempt

GRADE: 9

REPORTS TO: Director of Nursing for Clinical Services

INTRODUCTION

Under the supervision of the Director of Nursing for Clinical Services, the Infectious Disease Nurse Coordinator performs educational, clinical, and administrative activities in order to facilitate the delivery of primary and specialty health care to HIV-positive patients. This position works with the providers, clients, Unity staff members, and external referrals to ensure care is available according to Unity's standard of care.

MAJOR DUTIES/ESSENTIAL FUNCTIONS

  • Supervises HIV RN Care Managers and HIV Care Associates
  • Assigns HIV RN Care Managers to sites according to service needs.
  • Reports to the Department of Health women living with HIV or newly diagnosed with HIV who becomes pregnant
  • Ensures that the RN Care Managers follow and provide adequate support and education to HIV positive patients according to Unity's standard of care.
  • Collaborates with the interdisciplinary team to achieve patient and program goals.
  • Collects timely reports from HIV RN Care Managers and Care Associates.
  • Ensures HIV RN Care Managers and Care Associates get educational updates.
  • Works with site leadership to integrate HIV care into the patient's medical home.
  • Collaborates with and receives directions from the Chief of Infectious Diseases in implementing HIV quality improvement programs.
  • Actively participates on Quality Improvement committee.
  • In collaboration with the Chief of Infectious Disease, coordinates the monthly HIV Provider Meeting and maintains minutes for the meeting.
  • Provides oversight under the direction of the Chief of Infectious Disease of HIV auditing processes
  • Works as a RN Care Manager as needed.
  • Ensures improvement in practice settings by assuming responsibility for life long learning and demonstrates leadership skills in decision making and problem solving.
  • Performs other duties as assigned.

QUALIFICATIONS

  • Degree in Nursing (BSN required) and licensed to practice in the District of Columbia.
  • Current and active BLS or CPR certification required
  • 3 years of case management experience or 3 years of clinical nursing experience
  • Strong clinical background with experience in HIV/AIDS.

  • Excellent interpersonal skills.
  • Demonstrated ability and commitment to indigent clients.
  • Must be well organized, flexible, and action oriented

SUPERVISORY CONTROLS

The Infectious Disease Nurse Coordinator reports to the Director of Nursing for Clinical Services, and supervises the HIV, Nurse Care Managers and HIV Care Associates.

GUIDELINES

The position abides by all rules and regulations set forth by applicable licensing and regulatory bodies, as well as UHC policies and procedures.

PERSONAL CONTACTS

The Infectious Disease Nurse Coordinator will work with Unity providers and staff in Adult Medicine and Pediatrics.

RISKS

The position involves everyday risk and discomforts, which require normal safety precautions typical of such places as offices, meetings, training rooms, and other UHC health Care Sites. The work area is adequately lit, heated, and ventilated. The position requires contact with staff at all levels throughout the organization. There are also external organization relationships that may be a part of the work of this individual. All medical services shall be provided according to medically accepted community standards of care. The employee shall provide evidence of a recent (within the past twelve (12) months) health assessment that includes a PPD and/or chest x-ray results.
