Job Description
Under limited supervision the Clinical Lead precepts all new Access RN's and the team in general when new processes are executed in the department. Assists with he smooth and timely transition of all transfer patients from outside our service area and from other hospices. Serves as a resource to all Access RN's, Access Coordinators, and others needed throughout the organization. Serves as an admission nurse as needed up to 50% time.

Education

Diploma, ADN and/or BSN from an accredited institution

Experience

Five years of nursing experience. At least one year of Hospice admission experience preferred.

Licensures, Certifications

  • Current Maryland State license as a Registered Nurse

Physical Requirements

  • Ability to concentrate and pay close attention to detail often in stressful situations.
  • Ability to perform home care visits Generally sedentary work but may require standing and walking for approximately 10% of work time

Working Conditions

  • Normal office environment.
  • Maybe exposed to adverse working conditions due to performance of patient care activities and exposure to communicable diseases

Conditions of Employment

Principal Duties and Responsibilities

Human Resource Functions

  • Precept new Access Nurses Communicates any observed job performance issues to the Associate Director-Access.
  • Assists the Associate Director-Access in fostering a positive work environment as evidenced by staff morale, turnover rates and/or vacancy rates.
  • Participates in recruitment and retention activities as needed.
  • Holds self and staff accountable for outcomes.

Clinical/Educational Responsibilities

  • Facilitates a learning environment for all access RN's . Assists and guides staff in performing their direct care responsibilities.
  • Collaborates with f Access Director-Access to implement changes to clinical protocols, policies, procedures, and standard work, workflows; new programs and services to fill identified service gaps.

  • Provide Admission paperwork Training for Access Nurses and Access Coordinators.
  • Perform the duties of a Field Access RN as directed by Associate Director-Access to meet departmental needs.
  • Provides coverage for Access Care Transition- Nurse.

Daily Operations

  • Uses problem-solving and critical thinking skills to resolve basic clinical and operational issues.
  • Assists the Associate Director-Access in improving and maintaining compliance with regulatory requirements such as HIS, and CAHPS.
  • Acts as a resource for Access Coordinators and Access Nurses and the Quality Assurance Staff
  • Follow up phone calls to patients, families, and doctors as needed.
  • Act as a resource for the Access Nurses throughout the day as needed.
  • Works to meet or exceed all department performance metrics.

Service Excellence

  • Assists Access Associate Clinical Director with follow up and service recovery as needed and escalates as appropriate
  • Ensures exemplary customer service skills in every interaction both with internal and external customers.
  • Assists the Associate Director: Access in improving patient and referral source satisfaction.
  • Ensures referral source, patient and family concerns are addressed timely and that appropriate services/resources are accessed to address issues and concerns.

Communication and Coordination

  • Ensures that appropriate staff and management are kept up-to-date regarding patient admissions.
  • Coordinates the timely and safe transfer of out of area patients from other hospices and other healthcare institutions.
  • Assist with the timely and safe admission to either hospice or other Gilchrist or Gilchirst partner service. .
  • Escalates complex issues both nursing and physician through the appropriate chain of command.
  • Communicates as necessary with other departments to coordinate efforts, resolve operational issues and improve the quality of patient care.
